Medical Form:
Parental Release Form/Medical Form - You only need to fill this out at the beginning of each year and then we'll keep it on file. If your student has attended any event in September 2008 or later, you won't need to fill our the Parental Release Form again until the fall of 2009. Please call our office with any questions.
